Having your paperwork ahead of time saves us time in your initial session and provides me an opportunity to get familiar with your individual needs. Some questions may seem unrelated to your specific health concerns, but rest assured they are all important in the assessment of your overall health.
As my practice is by appointment only, your treatment time is set aside specifically for you. Because of this, I require 24 hrs notice for appointment reschedules or cancellations to avoid the following charges:
Cancellation or reschedule with 24 hrs notice or more = no charge
Cancellation or reschedule with less than 24 hrs notice = 50% of cost of session will be charged
Cancellation without notice or no-show = 100% of session will be charged and credit card will need to be on file for future appointments
